The School Wellness Ministry Program provides school health for the Catholic schools. The Health Questionnaire and Physician Certificate of Examination Form must be completed by the first day of school. The law in the State of Indiana requires that your child have certain immunizations in order to attend school. Please review the updated immunization requirements by clicking "Immunization Requirement Letter" to the left.
The State of Indiana recognizes only two reasons of non-immunization of children. One is medical. If your child has a medical reason for not being immunized, there is a form that must be filled out annually. A physician is the only health care provider that is able to sign this form. The second reason is religious. If religion is the reason, the parent must sign a form annually. These forms are due on the first day of school.
All medication must be kept in the health office.
Over-the-Counter Medication
If you want your child to receive any over-the-counter medications such as Tylenol, Motrin, Tums, etc. please send in a supply with your child's name marked on the outside of its original container and complete the Written Consent for Administration by Parent form with your signature.
Prescription Medication
State law requires that prescription medication be accompanied by written permission from both the parent/guardian and a doctor. See Written Consent for Administration by Physican form located on the right.
Bee Sting Allergies and Allergic Reactions
If your child has a bee sting allergy and requires administration of an Epi-Pen or Benadryl, these items need to be supplied by the parent and brought into the health office. A Bee Sting Allergies and Allergic Reactions form must also be completed.
